Sinus lift

When bone is lacking at the back of the upper jaw, a sinus lift makes it possible to place implants. Techniques, recovery and timescales.

Why bone is sometimes lacking

Above the upper molars is the maxillary sinus, an air-filled cavity. When those molars are lost, the bone gradually resorbs and the sinus tends to take up the space. Over the years, so little bone height may be left that an implant cannot be placed reliably.

What is a sinus lift?

It is a surgical technique that carefully lifts the membrane lining the inside of the sinus and fills the resulting space with graft material. Over time this material integrates and becomes the bone in which the implant will sit.

Two ways of doing it

  • Through the implant site itself (crestal approach): when only a little height is missing. The membrane is lifted from the hole where the implant is going to be placed and, often, the implant is placed in the same procedure.
  • Through a lateral window: when a lot of bone is missing. The sinus is accessed from the side of the upper jaw; depending on the height available, the implant is placed at the same time or in a second operation a few months later.

How it is planned

Always with a 3D dental scan: it measures the height and width of the bone and shows the condition of the sinus. If there is sinusitis or another problem with the sinus, it is treated first.

Recovery

There is usually swelling and sometimes some bruising on the cheek. During the first few weeks it is best not to blow your nose hard, to sneeze with your mouth open and to avoid strenuous activity and sudden changes in pressure, such as diving or flying, until the surgeon gives you the go-ahead. The graft needs several months to mature before the implant receives the final prosthesis.

Is it always necessary?

No. Depending on the case, shorter implants or implants in a different position may be considered, and in very severe atrophy of the upper jaw there are alternatives such as zygomatic implants. The decision is made with the full assessment in front of us.
